Endometriosia

  • Mechanical distortions caused by adhesions, endometriomas and repeated surgeries.
  • Altered immunological and the resulting inflammation.
  • Abnormal cytokines in the peritoneal fluid may cause sperm immobilization or interfere with sperm-oocyte interaction or decrease the oocyte quality and embryo quality.

Pathogenesis of infertility associated with endometriosis

  • The ovarian reserve is affected. Endometriosis and surgical treatment involving ovaries can also cause a decrease in ovarian reserve by causing a reduction in functional ovarian tissue.
  • Biologic mechanisms that may link endometriosis and infertility
    • Distorted pelvic anatomy
    • Adhesions impair oocyte release and/or ovum pickup
